Report of Game Misconduct and/or Match Penalty Form

The coach or manager of a participant receiving the Game Misconduct and/or Match penalty must file this incident report within twenty-four (24) hours. It is automatically sent to the DVHL Commissioner when you hit the “Submit” button.

All Match penalties involving Abuse of Officials must be reported to the Atlantic Affiliate Vice President of Protests, Suspensions, and Appeals, Mike Lichtenberger. You can forward your return acknowledgment email to Mike at [email protected]. This report is due for ALL Match & Game Misconducts received in League, Non-League, Playoff and Tournament games.

This form will be submitted for players violating the four (4) penalty rule in a game or for teams with twelve (12) penalties or more for a coach.

Failure to file this report within the specified time shall result in a fine of $30.00 on the organization of which the manager/coach who failed to file the report is a member.
